Copy of MBBS Pass (in English\/Hindi)<br \/>\nii. Copy of MBBS Pass certified by the Embassy \/Apostiled<br \/>\niii. Copy of Address Proof (Voter ID \/Passport \/Driving License)<br \/>\niv. Two Photographs (as per Para 5.6 of Information Bulletin)<br \/>\nv. Annexure B (Page no. of Information Bulletin)<br \/>\nvi. Copy of Date of Birth Proof (Certificate of Matriculation)<br \/>\nvii. Copy of Passport of the applicant (Front Page &#038; Last Page \u2013 Personal Information) \u2013 not mandatory for candidates who have done their MBBS from Nepal.<br \/>\nviii. Copy of 10+2 passing certificate<br \/>\nix. Copy of 10+2 mark sheet<br \/>\nx. Copy of internship certificate (if done abroad)<br \/>\nxi. Copy of eligibility certificate issued by Medical Council of India (if applicable)<br \/>\nxii.
